Sinus Rhythm
Let’s talk about sinus rhythm.
The term “sinus rhythm” is used when the rhythm originates in the sinus node at atrial rate in between 60-100 bpm.
Criteria
- Normal P wave with constant morphology.
- Regular atrial rate in between 60-100 bpm.
- P wave is upright in lead II.

There are two exceptions where P wave can be negative in lead II and may still sinus originin:
- Dextrocardia
- Limb lead misplacement.
Note: It is important to clarify that sinus rhythm can be present in the setting of 2:1 AV block, third degree or complete AV block, etc.
